Print of this Order Form and mail or fax it to us to place an order. Note, you may need to download Adobe Acrobat Reader in order to view and print the order form.

We require that your order arrives at our office a minimum of 10 days prior to the date you need your items to arrive at your home.

We can send your order to camp and your child will receive it when he or she arrives. If sending to camp, we require the order by the Wednesday before the child attends camp.

Note: There is no shipping charge if you choose to have the items delivered to camp.
